The Centers for Medicare & Medicaid Services (CMS) announced the Fiscal Year (FY) 2025 updates to the International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CM) coding system. Although fewer in number than some previous years, the FY 2025 ICD-10-CM code updates brought significant new classifications.
Note
For dates of service on or after October 1, 2025, use FY2026 codes. FY2025 codes should only be used for services during the FY2025 period (October 1, 2024 - September 30, 2025).

ICD-10-CM Coding Guidelines Updates
The 2025 ICD-10-CM Official Guidelines updates were minimal, primarily focusing on harmonization with new code introductions.
Status Code Updates
The title of category Z17 was expanded to "Estrogen, and other hormones and factors receptor status." This title change required guideline II.C.21.c.3 to be updated for consistency.
New Guideline: Presymptomatic Type 1 Diabetes Mellitus
A completely new guideline, I.C.4.a.1.(a), was added related to new codes for presymptomatic type 1 diabetes mellitus. The guideline indicates that codes from subcategory E10.A- are assigned for early-stage, type 1 diabetes that predates the onset of diabetic symptoms.
Key Point
The age of a patient is not the sole determining factor for type 1 vs type 2 diabetes, though most type 1 diabetics develop the condition before reaching puberty. Codes E10.A- are for early-stage type 1 diabetes that predates symptom onset.
Significant Updates by Chapter
Chapter 2: Neoplasms (63 New Codes, 14 Deleted)
The codes in categories C81-C84 now have an option to report "in remission." The codes in categories C86 and C88 now have options to report "not having achieved remission" or "in remission."
Chapter 4: Endocrine, Nutritional and Metabolic Diseases (16 New Codes)
Three new subcategories of codes were added:
- E10.A- Type 1 diabetes mellitus, presymptomatic
- E16.A- Hypoglycemia level
- E66.8 Obesity class (expanded specificity)
Several subcategories have new instructional notes directing reporting of additional codes for use of injectable non-insulin antidiabetic drugs (Z79.85) and/or new codes from subcategory E16.A- Hypoglycemia level.
Chapter 5: Mental, Behavioral and Neurodevelopmental Disorders (26 New Codes)
The codes in category F50 Eating Disorders for anorexia, bulimia and binge eating were expanded to specify severity. New codes were added specifically reporting PICA and rumination disorder in adults.
Chapter 11: Diseases of the Digestive System (27 New Codes)
Codes in category K60 Fissure and fistula of anal and rectal regions were expanded to specify complexity and nature of occurrence. Several instructional notes were added directing the assignment of codes for associated diagnoses.
Chapter 13: Musculoskeletal System (33 New Codes)
Codes for lumbar and lumbosacral intervertebral disc degeneration (M51.36 and M51.37) were expanded to specify type of pain as:
- Discogenic back pain only
- Lower extremity pain only
- Discogenic back pain and lower extremity pain
- Without mention of lumbar back pain or lower extremity pain
Excludes 1 notes were added excluding these codes from being reported with codes M54.3, M54.4 and M54.5. Code M65.9 Synovitis and tenosynovitis was expanded to include specification of site and laterality.
Chapter 19: Injury, Poisoning (30 New Codes)
Category T45 received 6 new codes to report poisoning by, adverse effect of and underdosing of immune checkpoint inhibitors and immunostimulant drugs. Code T81.32 was expanded to include four new codes providing greater specificity for disruption or dehiscence of internal operation or surgical wounds.
Chapter 21: Z Codes (25 New Codes)
Category Z17 was updated and 8 new codes were added to indicate positive/negative receptor status and/or combined receptor status. A new subcategory Z67.A- Duffy phenotype was added. Other updates included:
- Z15.- Genetic susceptibility to disease
- Z51.A- Encounter for sepsis aftercare
- Z59.7- Insufficient social insurance and welfare support
- Z68.5- Body mass index [BMI] pediatric
- Z83.72 Family history of familial adenomatous polyposis

Transition Period Considerations
When processing claims that span the transition between FY2025 and FY2026:
- Use the code set valid for each specific date of service
- Do not apply FY2026 codes to FY2025 dates of service
- Verify code validity when resubmitting denied claims from the prior year
- Maintain access to both code sets during transition periods
